Add something like the following to your extensions.conf:
exten => 452,1,SIPAddHeader("Alert-Info: <http://www.notused.com>\;info=alert-autoanswer\;delay=0")
Note: in order for this to work, the receiving phone needs to allow auto-connect/intercom calls: see setting "Answer After Policy".
